It still happens with teachers, though. There was one recent case where an elementary school teacher was blindfolding the kids and putting insects on their faces as part of whatever else it was he was doing.
He got caught because he was photographing his activities and when he took his film to be developed, the person doing the developing called the police. If this guy had been using a digital camera, he'd still be getting away with it.
And he's hardly the only one. I agree with you that it usually seems to be a family-member or a friend, but also teachers and people at church. Anywhere that there's a relationship of trust and the ability to be alone with the child, that's when it can happen.
