Surprisingly enough, I got 1.5 million returns when I Googled the phrase "paranoid that i smell."
I didn't check them all (sorry), but I didn't see anything very helpful.
It seems unlikely that you actually do smell. If your family is anything like mine, it's guaranteed they would tell you.
How are your sinuses? Perhaps you just have a chronic infection (sinusitis), and that's what you are sensing.
Of course, that wouldn't explain why you think you hear people talking about you. You know, rationally, that's not happening, right? A professional's opinion may be useful for you.
