In all honesty, TWATS asked one very key question... how old were they? If they are old enough to understand what sex is, and that means any type of sexual interactions including oral sex, then they probably should not be wrestling in their underwear, nor should they be seeing eachother Like that. But, if they don't know where babies come from and they don't know what sex is then no I wouldn't think anything of it but do make sure that they understand brother and sister are not supposed to behave such a way.
So essentially, if they're 4th grade above, they shouldn't be doing that.
