The term hillbilly has, for the most part, been watered down to mostly mean the same thing as redneck. At least from a general linguistics perspective.
On a technical level I believe you essentially just have to be from a rural, isolated community in the hills. The term is most commonly used to describe people from Appalachia for obvious reasons.
Redneck is a hateful word. Hillbillies are folks that live way outside of the cities. There are plenty in Pennsylvania and New York as much as there are in rural Mississippi or rural flanders Belgium.
